2007 Annual Health Care
Facility Surveys
The Department of Public Health and Human Services has made available on-line all annual health care facility surveys, and the Word survey may be completed and returned electronically to consurvey@mt.gov. The health care facilities required to complete an annual survey are:
Ambulatory Surgical Centers
Critical Access Hospitals (CAHs) and Hospitals
Home Health Agencies, and
Long-Term Care Facilities
The ability of the department to keep records on health care facilities is essential. The information collected each year is accessed by many people across the state and the nation, and is utilized by the Certificate of Need Program to determine patient need in communities around the state. Because the information in this survey may affect the health care services in your community, it is important to be as accurate as possible. In addition, State of Montana law requires that health care facilities supply such information.
