2017 Certificate of Need Annual Health Care Facility Reports
The following health care facilities are required to complete an annual Certificate of Need Health Care Facility Report:
Ambulatory Surgical Centers
Critical Access Hospitals (CAHs) and Hospitals
Home Health Agencies, and
Long-Term Care Facilities
The Department of Public Health and Human Services has made all annual Certificate of Need health care facility reports available on-line. It is suggested that you first print the report and fill in the questions before completing the online version.
The reports have a new format this year. The links to the instructions and blank report form can be found on the first screen of the reports.
The ability of the department to keep records on health care facilities is essential. The information collected each year is accessed by many people across the state and the nation. Data is utilized by the Certificate of Need Program to determine patient need in communities around the state. Because the information in this survey may affect the health care services in your community, it is important to be as accurate as possible. In addition, State of Montana law requires that health care facilities supply such information.
