2016 Certificate of Need Annual Health Care Facility Reports

The health care facilities required to complete an annual survey are:

  • Ambulatory Surgical Centers

  • Critical Access Hospitals (CAHs) and Hospitals

  • Home Health Agencies, and

  • Long-Term Care Facilities

The Department of Public Health and Human Services has made available on-line all annual Certificate of Need health care facility reports. It is suggested that you first print out and fill in the questions to help you fill out the online version.

The ability of the department to keep records on health care facilities is essential. The information collected each year is accessed by many people across the state and the nation. Data is utilized by the Certificate of Need Program to determine patient need in communities around the state. Because the information in this survey may affect the health care services in your community, it is important to be as accurate as possible. In addition, State of Montana law requires that health care facilities supply such information.
