Passport to Health and Team Care

Passport to Health is the primary care case management (PCCM) program for Montana Medicaid and Healthy Montana Kids (HMK) Plus members. Team Care is a program for Montana Medicaid and HMK Plus members where they are assigned a medical team (one provider and one pharmacy) that they are required to use for all their prescriptions.

The Passport to Health and Team Care programs encourage and support Medicaid and HMK Plus members and providers to establish a strong doctor/patient relationship and ensure the appropriate use of Medicaid and HMK Plus service.
