Aging Horizons is a public/private partnership that meets DPHHS's Senior and Long Term Care Division’s State Unit on Aging’s Federal requirement to serve as an effective and visible advocate for older adults, to disseminate information on the needs of older adults, and to utilize outreach efforts that will help to identify and inform individuals about their eligibility, or their parents' eligibility, for various assistance programs.
