Online Lab Portal Procedures and Access Forms
Montana Public Health Laboratory test ordering and results are available through a secure online lab portal. MTPHL Laboratory Information Systems (LIS) staff has held multiple online demonstrations to familiarize our partners with online test ordering. We are no longer providing paper requisitions. We have created some training documents for your convenience:
- Order Entry: including creating a new patient record, creating a manifest, canceling and adding on tests, entering medicaid information, and creating tickets for providers not in our system.
- How to search for tests (during order entry)
- Retrieving results: including accessing location inbox, changing your password, accessing patient reports and records, printing lab reports, viewing linked documents and viewing pending orders.
- Creating a General Client Services Ticket
- If you are making a change to one of your unique patient identifiers, after the patient sample has arrived at MTPHL, please fill out this form and link it to a ticket. Unique Identifier change request form.
- Creating a Supply Request
For access to our lab portal please read and fill out these forms below.
- The laboratory supervisor should fill out and sign the Access Authorization Form.
- The User Attestation is to be filled out by every individual that needs access to our system.
- Facilities that have 10 or more users have the option to have a Local Admin Attestation.
