Home Birth FAQ

If the midwife is licensed and certified with the State of Montana, the midwife will be responsible for submitting the information to our office. Even though the baby was born at home or in a birthing facility, in this scenario, the homebirth will not be processed by the state. However, if the midwife is not licensed and certified with the State of Montana, the parents ARE responsible for filing the home birth.

The more complete and accurate your paperwork is, the sooner it will be processed. Providing your contact information (email and/or phone number) will speed up the process if we need to contact you. Other internal factors also affect processing time. Once all information has been received, these typically take up to two weeks to process.

If your baby was born within the current year or no earlier than July 1 of the prior year, be sure to select “Yes” when asked for permission to provide information to the Social Security Administration on the Home Birth Worksheet.

You can contact the Social Security Administration at 1-800-772-1213 to check the status. Per SSA’s website, your card should arrive within 14 days after the application has been submitted. The submission from our office to SSA can take up to one week after the birth certificate has been created.

The Montana Immunization Program has a database called imMTrax. This database is confidential and stores immunization records for Montanans of all ages. It helps to keep all those records in one place for convenience when you may need it for situations such as enrolling your child in school, or employment. The top portion of the Home Birth Worksheet is required to build the birth certificate. The bottom half is for statistical purposes only and is provided to the CDC in aggregate format. The information is used by the CDC in determining and developing funding needs for such things as educational and pre-natal support programs as well as identify birthing trends.

Please be sure you are providing proof of residency for the address where you lived at the time of your baby’s birth. You can do so with any of the documents from page 2 of the Home Birth Packet.

Yes, a Paternity Acknowledgement form will need to be completed and signed by both parents in front of a notary and returned to our office to add the father to the certificate.

A Statement of Non-Paternity form will first need to be completed and signed by the mother and her husband in front of a notary. Then, the mother and the child’s father will need to sign a Paternity Acknowledgement in front of a notary. Both forms must be returned to our office.

The listed father will need to complete the Notice of Withdrawal of Paternity Acknowledgment and sign it in front of a Notary then return it to our office.
