How to Add Services to a Current DDP Provider Contract

For a qualified provider of DDP services with current contract to be authorized to provide additional services -

• Determine which additional services the provider is able to provide under a particular funding source. See the links under DDP Medicaid Waiver Information for the specific waiver(s) and applicable Administrative Rules of Montana (Dept. No. 37, Chapter 34).
• Once the provider determines which additional services that can be offered under Department rules, the provider must fill out the Current Provider application. On this application, identify additional services the provider is interested in providing.

For existing DDP qualified providers to add licensed/certified services to a current contract with the Developmental Disabilities Program (DDP), the provider must use the application to add a licensed/certified service.
