The purpose of this page is to compile resources for Adult Mental Health and Substance Use Disorder providers a central location of relevant resources to provide an easy access to information.

State Approval

Application Materials

  • State Approved applications must be submitted to bhddapprovalapps@mt.gov
    • All materials must be submitted that are indicated on the application.
  • Substance Use Disorder Facilities (SUDF) must begin their licensing process first prior to applying to state approval with the Office of Inspector General.
    • Click on "Bounds" Licensing System- Provider Portal LOGIN to begin the process.
  • All programs must start their initial Medicaid enrollment at Optum. 
    • Individual LAC's must enroll as a Provider Type 77.
    • SUDF's must enroll as a Provider Type 32.
  • Medicaid billing CAN NOT begin for state approved programs until they have completed the state approval process.
