Certification Bureau Survey Results and Plans of Correction

For best search results, it is recommended to search by town/city. When searching by facility name, partial name searches may provide better results.

The Certification Bureau is responsible for conducting surveys to ensure Montana facilities that accept Medicare and Medicaid payments meet federal regulations required by the Centers for Medicare and Medicaid Services (CMS).

This search shows survey results for most surveys conducted by the Certification Bureau. This site has the three most recent recertification surveys (health and life safety code) and any revisit or complaint surveys conducted during that time. If you wish to view a prior survey, please submit your request to mtssad@mt.gov

Health Surveys observe quality of care; quality of life; safe and sanitary conditions; and patient/client/resident rights.

Life Safety Code (LSC) Surveys determine the facility's safety and adherence to the National Fire Protection Agency code.

Complaint Surveys are conducted after the Certification Bureau receives a complaint.

Revisit Surveys verify correction of deficiencies cited in a previous survey.

The Statement of Deficiencies (SOD) report the survey findings of the Certification Bureau.  SOD will be released on the 15th day after the document is available to the facility, which is 25-30 days after the survey.

The Plan of Correction (POC) includes the (SOD) as well as the facility's response on how it plans to correct the deficiencies. The POC cannot be released until it has been reviewed and accepted by the Bureau, therefore the time frame in not automatically set.

The Certification Bureau surveys the following facility types: Hospitals, Nursing Homes, Ambulatory Surgical Centers, Rural Health Clinics, Home Health Agencies, Hospices, End Stage Renal Disease, Psychiatric Residential Treatment Facilities, Outpatient Physical Therapy Providers and Intermediate Care Facilities for Individuals with Intellectual Disabilities.
