See the DD Rates page for the Manual and Summary Rates sheet.  The manual is also on the Medicaid Provider site.  The Medicaid Provider site will also have a fee schedule.  It will look slightly different from the DD version.  Keep checking the Summary Rates sheet for updates as we work through things!
