Primary Care Montana (PCMT)
Primary Care Montana (PCMT), a new primary care case management (PCCM) program designed to modernize and simplify health care delivery for providers and members across the state. PCMT is a value-based primary care model designed to promote preventive care and optimize coordination for Montana Medicaid and Healthy Montana Kids (HMK) Plus enrollees. The program establishes a "glide path" for providers toward advanced population health management while maintaining budget neutrality for the state.
Next Steps
The program will be rolled out in phases to ensure a smooth transition:
- Tiers 1 and 2 (Effective July 1, 2026): Focus on improving quality metrics, increasing post-hospitalization follow-ups, and reducing readmissions.
- Tier 3 (Targeted July 2027): This advanced tier will focus on high-risk care management and offer opportunities for shared savings.
- Technical Assistance: Beginning in April 2026, the Department will provide technical assistance to support providers during the "go-live" period.
