Extended Facility Information

The information on this page is to be utilized by facilities that currently have an extended registration / license.

Facilities that wish to apply for an extended registration / license must contact their Child Care Licensor in writing.

Extended Facilities must provide the following documentation annually:

• Extended Attestation Document
• Extended Attestation Staff List

Extended Facilities must provide New Employee Information within 15 days of hire.

All documents for Extended Registrations/ Licenses must be submitted to:
