AFIX awards are presented for achievement in immunization coverage levels based on the assessment conducted during the 2017 VFC clinic review.  

 

90% Award

The 90% Award is presented to clinics with at least 11 records that achieve a 90% or higher UTD coverage rate for the series. This year the award is based on 4 DTaP, 3 Polio, 1 MMR, 4 Hib, 3 Hepatitis B, 1 Varicella, and 4 PCV (4:3:1:4:3:1:4) series.

Certificate of Achievement – Healthy People 2020

The Certificate of Achievement is presented to clinics with at least 11 records that achieve the Healthy People 2020 goal of 80% or higher for the 4 DTaP, 3 Polio, 1 MMR, 4 Hib, 3 Hepatitis B, 1 Varicella, and 4 PCV (4:3:1:4:3:1:4) series.

The Improvement Award is presented to the top five, by clinic size, most improved practices over a three year period for the 4 DTaP, 3 Polio, 1 MMR, 4 Hib, 3 Hepatitis B, 1 Varicella, and 4 PCV (4:3:1:4:3:1:4) series. Practices must have at least a 60% UTD coverage rate for the series.
